If you’ve struggled with sorting out the root cause of a problem in the past, you might have wondered, “Is there a 5 Whys Excel template that will make problem-solving easier?”
Yes, there is. Excel is an excellent tool for working through the problem and conducting a root cause analysis. The worksheet grid makes it easy to organize information about the problem and sort through issues, so you can solve the problem and keep it from happening again in the future.
In this article, I’ll show you the 5 Whys Excel template I use to get to the root cause of a problem and come up with a solution to solve the problem. Using this template can also help you figure out how to stop the problem from returning weeks, days, or months from now.

The screenshot above shows you the first page of the template. At the top, you’ll fill in summary details about the problem. You may notice that the form doesn’t show the cell gridlines that you usually see in Excel. If you want to display the gridlines, go to the View tab and select the Gridlines option.
Next, in the middle section, you complete the process of asking the 5 why questions. Simply type your answers in the space provided.
If you figure out the root cause of the problem before you get to the 5th why question, just stop, and move on to the next section.
Once you get to the last section, describe your plans to deal with the problem now and what you will do to prevent the problem in the future.
Next, take a look at the screenshot below. This is the second page of the 5 whys template. This page is helpful for recording notes related to the problem or solution.
The next screenshot shows the instructions page. The instructions are short and easy to follow.
The final screenshot below shows a completed example. While I used a work example, the 5 whys method is just as effective when dealing with problems outside of work. Use the notes in the completed example for inspiration or to guide you through the process.

How to Copy the Blank 5 Whys Excel Worksheet
Copying the worksheet is easy. To copy the worksheet:
Press the CTRL key on your keyboard, then drag the sheet tab to the right. You can also right-click the worksheet tab and choose the Move or Copy… option.
